At Wheeler Creek CDC, our commitment to the Permanent Supportive Housing Program is rooted in compassion and proven success. Program Components
Our PSH program targets individuals and families who have experienced long-term homelessness and have documented disabling conditions. Participants receive ongoing rental subsidies, ensuring access to secure housing. Our experienced case managers provide crucial supportive services, including mental health counseling, substance abuse treatment, employment assistance, and connections to community-based health care. Over 80% of eligible DC residents are Medicaid-eligible, and we actively enroll participants to improve access to necessary services.
Housing Navigation
WCCDC utilizes a comprehensive housing navigation approach to swiftly transition individuals and families from homelessness to permanent housing. We employ outreach and engagement strategies through the Homeless Management Information System (HMIS) and community partnerships to expedite housing placement. Our strong rapport with landlords and access to rental resources ensure clients have diverse housing options that meet their needs and preferences.
Client Journey
The individuals and families enter our PSH program through the Coordinated Access Housing Placement (CAHP) process, prioritizing needs-based housing placement. Upon enrollment, each participant is matched with a dedicated case manager who conducts a comprehensive assessment and develops an Individualized Service Plan (ISP) within 30 days. This plan guides them through housing identification, move-in support, and ongoing stabilization efforts. Housing Stabilization
Once housed, our focus shifts to housing stabilization through intensive case management. Case managers assist clients in maintaining tenancy responsibilities, accessing community resources, and integrating into their neighborhoods. We provide ongoing support to address health, mental health, income, and other barriers to stability. Regular assessments and service plan reviews ensure participants receive continuous care tailored to their progress and changing circumstances.
